• EMDR

    From www.emdria.org:

EMDR is a structured therapy that encourages the patient to focus briefly on the trauma memory while simultaneously experiencing bilateral stimulation (typically eye movements), which is associated with a reduction in the vividness and emotion associated with the trauma memories. Eye Movement Desensitization and Reprocessing (EMDR) therapy is an extensively researched, effective psychotherapy method proven to help people recover from trauma and PTSD symptoms. Ongoing research supports positive clinical outcomes showing EMDR therapy as a helpful treatment for disorders such as anxiety, depression, OCD, chronic pain, addictions, and other distressing life experiences (Maxfield, 2019). EMDR therapy has even been superior to Prozac in trauma treatment (Van der Kolk et al., 2007).

Brief Rundown of what EMDR can look like in therapy:

    • EMDR has eight total phases and there is no specific timeline as far as how long each phase may take
    • The beginning phases of EMDR start as soon as our first appointment when I learn more about your story and what you'd like to work on
    • Eventually we will collaborate and pick a target, either a specific memory or core belief, to reprocess
    • You will use my light bar/buzzers to utilize eye movements/bilateral stimulation to move these memories from one part of the brain to another
    • The goal is for the memories/beliefs we work on to feel neutral and objective once all eight phases of EMDR are complete
    • There is an option for us to do extended EMDR sessions (90 minutes, 3 hours, or 4 hours) and even intensive sessions (3-4 hours multiple days in a row). We will decide together if these extended/intensive sessions are a good fit

    KETAMINE-ASSISTED PSYCHOTHERAPY (KAP)

Ketamine-Assisted Psychotherapy (KAP) is a holistic modality in which ketamine is used as a complement to psychotherapy
to help eligible clients experience more frequent breakthroughs and sustained improvement in symptoms. I take on the psychotherapy portion of the experience, while Journey Clinical’s medical team supports you on all medical aspects. This includes determining eligibility, developing a custom treatment plan, prescribing the medicine and monitoring outcomes. Below is more information about KAP to help you navigate if it may be a good fit for you.

    What Is Ketamine?

Ketamine is a legal, safe and effective medicine used to treat a variety of mental health conditions, including depression, anxiety and PTSD. Ketamine has rapidly acting antidepressant and mood-enhancing effects, which can begin to take effect within 1-2 hrs. after treatment. It works by blocking the brain’s NMDA receptors as well as by stimulating AMPA receptors, which are thought to help form new synaptic
connections and boost neural circuits that regulate stress and mood. Ketamine has also been shown to enhance overall neuroplasticity for lasting symptom improvement. Ketamine can be administered in a variety of ways, including IV infusion, intramuscular injection, via nasal spray and using sublingual lozenges. In my work with Journey Clinical we only use the sublingual lozenge form.

    How does Ketamine feel?

The effects of ketamine, which most clients find pleasant, last for approximately 45 minutes. These effects can make you feel “far from” your body, and facilitate shifts in perception that can often feel expansive in nature. Your motor and verbal abilities will be reduced, so you’ll be lying down in a comfortable position during the experience. Once these effects subside, we’ll spend the remainder of our appointment giving you space to process and discuss your experience. While it may feel hard to articulate what happens during the experience, clients feel like the insights gained are clear.

Brief rundown of what KAP can look like in therapy:

    • After we meet and you decide you'd like to utilize KAP in our work together, I will refer you to Journey Clinical
    • You will meet with one of Journey Clinical's psychiatrists/nurse practictioners for a 1 hour intake appointment (separate cost of $250/intake & $150/follow-ups)
    • Ketamine will be sent directly to you and then you bring the medicine in to my office for a dosing session
    • Dosing sessions are typically 2.5-3 hours long
    • We will meet for an integration session within 1 week of your dosing session
    • Typically the treatment plan for most clients is 6-8 dosing/integration sessions total
    • An additional option is KAP groups once a month that include holistic modalities such as sound bath, yoga and reiki. This is a unique way to utilize KAP at a more affordable price as it includes prep/dosing/integration in a 5 hour session. Email sharonmburgamy@gmail.com to see which of these is the best fit for you!
